The situation

The procedure says one thing. The training says another. Neither is wrong enough to trigger a document change, and together they are wrong enough to produce a deviation - because the operator on the floor learned the version that no longer matches the step an investigator will read.

Most facilities arrive here honestly. Training was built from an earlier revision of the SOP and never caught up. Or the training was never really built at all: the procedure was assigned as read-and-sign, and “training” means a signature confirming the person scrolled to the bottom. Every SOP revision quietly breaks the training that references it, and no mechanism exists to notice.

Read-and-sign is the deeper problem. A procedure is written to control a process, not to teach it. Handing it to a new hire and collecting a signature produces a record of reading, not an ability to perform. When the deviation comes, the file will show the training was “current” and the investigator will ask what it actually taught.
